Clarke made just 11 from 20 balls before being caught behind when he gloved an attempted pull shot to wicketkeeper Quinton de Kock from the bowling of Dale Steyn.
The cricketer has battled a degenerative back problem and hamstring issues in recent years.
He missed the one-day matches against Pakistan in the United Arab Emirates and also returned home early from the previous limited overs tour of Zimbabwe because of the same problem.
The latest setback comes at the start of the five-match One Day International series against South Africa's Proteas and must place him in doubt for the remainder of the matches.
More significantly, it comes just three weeks out from the first Test against India, and only months away from the next World Cup.
